The Problem

While in-house communication is extremely important, healthcare is often a regional activity and quality medical care depends on critical communications with external stakeholders like long-term care facilities, home healthcare workers or partner medical facilities in the region. Most clinical communication platforms cannot handle all of these types of communications and remain HIPAA compliant. They address only part of the issue, forcing the addition of other platforms that add costs, or even worse, manual processes that leech critical time from a clinician’s day.

The Solution

Diagnotes helps team members connect with any clinician, caregiver, or even patient from anywhere. With Diagnotes Connect, users of Diagnotes in one health organization or network can easily connect and collaborate with users of Diagnotes in another organization, via HIPAA-compliant messages, calls, video chats and file sharing. Connect facilitates collaborative relationships with referral partners and makes transfers quick and easy.

Diagnotes Extend enables non-Diagnotes organizations to communicate directly with a Diagnotes organization. Requests to share information from the non-Diagnotes organization are quickly routed to the correct targeted care team member within Diagnotes. The ability for a video chat to be initiated is also provided. Extend keeps all communications secure and promotes collaboration even with non-users of Diagnotes.

Client Experience 

Diagnotes enables secure, remote consults — essential for both large, urban health systems and rural or suburban facilities, to provide telehealth services. A nephrology practice for a large IDN utilized Diagnotes live video chat to consult with an affiliated regional medical center. Clinicians at the partner facility, more than an hour away from the IDN’s main campus, would consult via secure video with specialists to determine if a transfer was needed.

The IDN’s specialists assessed the factors in the case, reviewed test results and spoke face-to-face with the patient and the local care team about the transfer decisions. The patient benefited by having a specialist “in the room.” In nearly all cases, the patients were able to remain at their local hospital, avoiding a costly and disruptive transfer. The affiliated medical center was able to continue providing care to the patients, the IDN was able to bill for the consultations and the patient was able to stay closer to home and near their families. In the event a transfer was warranted, the receiving facility was already briefed on the details of the case and began preparing an appropriate care plan prior to the patient’s arrival.
